[MaxLite] 401 Imaging ASA OON notice letter and Alternative Radiology list

Attached the letter of notice  that 401 Imaging is not ASA PPO network
provider  

Please do not to use 401 Imaging as if you do  you will be responsible for
the bill as an out of pocket expense. 

Alternative radiology  for Bergen county, NJ and Rockland county, NY are
listed on the excel spreadsheet

 

All individual policy holders and providers will be contacted regarding
this notice.
