[MaxLite] Annual health insurance

To all:

We are in the process the annual health insurance renewal and need your
assistance. 

As we look  at the various plans there are question regarding deductible and
MRIs, Cat, PET X-Ray.

We are looking for information regarding anyone who had in the past year or
gets any type of routine  MRI, Cat,Pet X-rays.

 

DO NOT need to know why you had to get it,  just  that you did.  The
response is totally voluntary and the information will be keep confidential
once we have a total the responses will be deleted.

 

If you can let me know between today and tomorrow I would appreciate it.
