Subject: [MaxLite] Medical Premiums
To all associates
For those of you that are enrolled in the Medical plan the attached file has
the new medical premiums by coverage type. Those of you rolled in Dental
already know those premiums.
In addition, MaxLite will reimburse you for deductible charges (i.e.
MRI.,CT scan, etc.) up to a maximum $750.00 per family.
In order to receive a reimbursement, the form included on the attached must
be completed along with a copy of the insurance company's Explanation of
Benefits (EOB). This reimbursement only covers deductible, which means your
office visit co-pay, RX and a like are excluded.
For the next 7- 10 days the insurance company will setting up the networks
and sending the ID cards shortly their after.
In the event of an emergency during this period, please contact myself or
Todd and we will work the insurance in getting you verification of coverage
for the provider.
