[MaxLite] Flu Shot

To All,

 

Our insurance agent will arrange flu shot for NJ employees as follows:

 

1)      Sept 23  12-2PM

2)      Must bring your ID card (insurance card and driver license or
passport)

 

 

Special Notes:

 

1)      Out of State employees- you can get such shot from your own doctor
with $20 copay

2)      To NJ employees- our agent has a special relationship with one who
administers this shot- so no copay. 

3)      To NJ employees who do not participate in Maxlite group insurance- I
exercised some influences over the agent. So you can get a free shot as
well.  You may pay me J
