[MaxLite] New Cigna Cards for Select Members - PLEASE READ!!!

Good Afternoon:

If you are a covered member of the MaxLite medical plan PLEASE read the message below/attached!!

An important message from Cigna to all MaxLite members residing in New Jersey, Colorado and Georgia:

It was recently discovered that your current member ID card does not include a line item that discloses
your hospital benefit information. It is required under NJ, CO and GA state law to include this information
on your insurance ID card. As a result, Cigna will be reissuing new ID cards to all members who live in
these three states. Note, if you receive a new ID card, it is intended to replace your existing ID card and
will now include your hospital benefit information. Nothing else on the ID card will change.
